This page provides access to Roundtable forms below.
You can use any browser you want, but you must use Adobe Acrobat Reader to open the forms. Adobe Acrobat Reader is free to download and install from this site: click here. Once installed, follow this 3-step process, to use the forms.
Download form – hover the cursor over the form name, right click and “save link as…” and save the document to your desktop.
Open form – Locate the document on your desktop, right click the form and open with Adobe Acrobat Reader
Process the form – Print the form or key the required fields in the form and submit it.
Forms highlighted in blue are standard forms. They can be printed, filled out manually and mailed to the address on the form.
Forms in green are “fillable” forms, online entry forms. Type the necessary info, click the SUBMIT FORM button on the upper right purple bar and the form is automatically emailed to RoundtableForms@aol.com via your Microsoft Outlook email applicaion.
If you do not use Microsoft Outlook, or if you have any problems with the submit process:
type the necessary information in the form,
save it to your desktop, and
email the saved form as an email attachment to RoundtableForms@aol.com using your normal email tool, such as Gmail, Yahoo, Verizon.net, etc…
Note: Instead of mailing a check, you can pay application/registration fees online at the Roundtable Store through secure and safe Paypal. A Paypal account is not needed. We accept credit cards!
